An experienced adults services and health leader is required to undertake a diagnostic review of hospital discharge and adult social care flow arrangements, identifying barriers to timely discharge and driving sustainable improvement across the wider health and care system.

Working closely with senior health and care partners, you will analyse complex operational challenges, develop improvement plans, redesign pathways, and implement practical solutions to improve patient flow and outcomes.

The successful candidate will bring substantial experience of hospital discharge, discharge-to-assess or system flow improvement programmes, alongside strong stakeholder engagement, analytical skills, and a proven track record of delivering measurable performance improvements.
